Now what is Patient Rewards Hub? Patient’s Rewards Hub is a special program that gives Y-O-U the ability to get rewarded simply for having fun.

Follow these easy steps to start joining the fun!
Step 1: To access our Patient Rewards Hub page, click here.
Step 2: Click on “Register”.
Step 3: After you click on register, you will be prompted to register a new account. Choose from the bottom left “I Never Received A Number or Code”.
Step 4: Almost done! Next, you will need to choose from the following options, then click “Next”.
Step 5: Now it is time to choose how you want to log in! Pick one of the following and then create your username and password. After you have completed this step, you will be registered and ready to start the fun!
